Leading With a Clinical Lens - ACHE Congress

In every field, there are times when the question "Who is running this place?" becomes commonplace. Who are our leaders, and what are their backgrounds? In the healthcare industry, many avenues lead to the C-suite. One challenge in healthcare is having leaders who truly know what is going on at the patient-care level. A parallel challenge is the concern that too many decisions in healthcare are driven solely by finances. In most healthcare organizations, the majority of the C-suite does not have clinical backgrounds. Many of these leaders have little in-depth exposure to patient care. What might (should) they do to enhance their understanding of healthcare's core business—caring for patients? A key issue non-clinical leaders often face is credibility. How often have clinicians remarked, "Administration doesn't even know what we are doing here." How can nonclinical leaders raise their awareness, competency, understanding and credibility from a clinical knowledge standpoint (short of going back to school to earn a clinical credential)? Moreover, even clinicians who move into leadership roles are often accused of being out of touch. Physician leaders are often called "suits," and other clinician leaders can get embroiled in the day-to-day aspects of administrative management and lose touch with the field. Finally, as health systems get larger, the tendency to have leaders in remote, non-clinical settings becomes more commonplace. This session will provide specific suggestions on how nonclinical leaders can enhance their knowledge of patient care on the front lines. Recommendations will also be provided on how clinical leaders can maintain their ability to remain in touch with front-line work.

Physician Leadership: Daunting Issues and Hurdles

There is little disagreement that more physician leaders are needed in the field. Yet, physician leaders face several unique hurdles that other healthcare leaders do not. They often have unclear or conflicting roles. Is the role of the physician leader to “represent” the medical staff in a stewardship manner or are they representatives of administration—or both? How can physician leaders balance their clinical work with leadership duties? Do many of the learned attributes from medical school and training conflict with what is required for collaborative team leadership?

Organizations usually promote physicians to leadership positions based upon their clinical expertise, and this is done typically when physicians are in their 40s, years later than most other C-suite leaders. What they often learned in medical school was to give orders and that top-down leadership style turns the team into sheep instead of skilled, collaborative team members. Physician leaders are asked to be “police officers,” monitoring and correcting poor behavior with other physicians without having the full authority to do so. One of the greatest differences between clinical practice and leadership is that, as a leader, you often will not know for years whether the path you chose was the best path or the right path. To be an effective leader means learning to live with ambiguity. This session identifies the key hurdles physicians need to pass to become effective leaders. Specific suggestions will guide organizations in developing physician leaders and how to address the obstacles and nuances that make the transition from clinician to physician leader difficult.

  • Identify and gain a deeper understanding of the obstacles that make the transition from clinician to physician leader so demanding.

  • Learn how to design a physician leadership assessment and development program so it is more practical, effective and hands-on.

This program explains how physician leaders often work in an “in-between” world where their roles and functions are blurred and unclear. Two factors cause this: ambiguous roles and the demands of adaptive challenges. Participants will learn to recognize these challenges and discover techniques to prevent them. With the significant growth of physician leaders comes the question of what or for whom do physician leaders represent? Are physician leaders merely the “chief union stewards” for physicians, representing and advocating solely for the rank-and-file clinical physicians? Or, are they representatives of administration and the C-suite, functioning as organizational leaders who create and endorse broad policy and execute strategic plans? Or, is there a middle ground in which physician leaders serve as intermediaries or envoys, helping to mediate conflict between two opposing parties? Clearly, physician leaders walk a fine line and the ability to carve out a clear role and play it well is critical to the success of all physician leaders.

Physicians have always been at the forefront of healthcare; essentially, they are the ones who drive most of the decisions that pertain to care, quality and costs. The ability of physician leaders to reshape organizational structure; to ensure that authority and responsibility are clearly defined; and to ensure optimum quality, safety, and cost management is critical. These are adaptive challenges. Organizations face two types of challenges: those that are technical, where a clear roadmap to follow exists, and those that are adaptive, which provoke unease, stress or anger. Adaptive challenges have multiple solutions and often upend long-standing traditions and deeply held beliefs. Addressing them takes time and usually a good deal of dialogue about what is changing and why. Physician leaders must become experts at managing adaptive change. They can do this by better delineating their roles and ensuring that the physician collective understands the need for increased physician leadership and how it will help them in their clinical practices.

  • Discover how to address adaptive challenges and gain credibility by helping groups come together, hash out divergent views and make decisions collectively.

  • Explore the importance of assigning authority commensurate with responsibility to reduce ambiguity and accountability.
